PCOC assists after new diagnoses, hospital discharge, or when caregivers need additional support. Examples include choosing between Medicaid waiver-funded in-home services versus nursing facility coverage.

Michigan Medicare Card and Identification Updates
By 2019, all Michigan Medicare beneficiaries received new cards with Medicare Beneficiary Identifiers replacing Social Security Number-based IDs. Since January 1, 2020, only MBI cards work for billing.
The new cards removed the SSN, gender, and signature line to prevent identity theft. If your card is lost or stolen, contact the Social Security Administration at 1-800-772-1213 or visit Medicare.gov to request a replacement.
